Full-time: 40 hours per week, 1.0 FTE (Days)

Description:

The Admission Coordinator will provide information about services to potential Seasons Hospice patients and families, and coordinate admissions into our hospice program.  This position will also assist with admission assessment and planning to ensure a smooth transition of new patients to the appropriate RN- Patient Care Coordinator.

Applicants must possess a minimum of a bachelor’s degree in social work, psychology, human services, or a related field. A Bachelor’s degree in Social Work is preferred.
